Library Card Application

This application is for adult (age 18 and above) Fayette County residents who are first-time cardholders. Lost cards may be replaced at any Lexington Public Library location. Please report lost or stolen library cards and any address changes as soon as possible. Your library card will be sent to you in the mail.

Please allow 10-15 weekdays for processing and delivery of your card.

Upon reciept of your library card, sign it using a felt tip pen. Your signature indicates agreement to the following:

I agree to observe all rules established by the Lexington Public Library and will be responsible for all material(s) borrowed on my card. I also agree to pay any fines or other charges imposed for late, lost, or damaged library materials.

I understand that all the above information will be kept confidential.

You will be required to present a photo ID the first time you use your card.
