Teacher Materials Request Form

The Lexington Public Library is dedicated to partnering with all schools in Fayette County to ensure the success of children. We gather and hold materials that support public, private, and parochial teachers’ needs. The Teacher Materials Request Form is designed to make this process more efficient. Please provide us, via this form, as much information as possible on your subject (i.e., "space travel", "friendship", etc.) so that we can select materials most suitable for your curriculum.

The online catalog is the most efficient way to request specific titles. For assistance with the catalog or this form, please contact any Library location.

Request Procedure:

Requests for materials require a minimum of 3 days for processing. Materials may be picked up at the location of your choice after this time. Please keep in mind that they will only be held for 7 days. Library staff will contact you if there is difficultly in filling your request or for clarification; for example, materials on the requested subject could be unavailable or checked out.

Materials are selected and pulled with the idea of supplementing a teacher’s lesson plan. For in-depth subject collections or materials for an entire classroom, please visit the library for personalized assistance.
